The pharmacist will provide quality, timely and cost effective drug therapy to our patients. This person will: Provide pharmaceutical care to patients, coordinate drug distribution, ensure medication safety, oversee technical staff and develop & maintain professional relationships with other healthcare providers. This person will work both decentrally on patient care units and in the central pharmacy.
Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
Provide clinical pharmacy services to all patients in assigned area using Standard Operating Procedures (SOP). This includes dispensing of medication, completing review for all patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.
Act as a resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure appropriateness of medication orders and therapy, drug selection, formulary correctness and compliance with other formulary or departmental initiatives as appropriate.
Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.
Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences and actively participating in department clinical meetings and activities.
Participate in regulatory, quality assurance and process improvement activities as required.
Meets all regulatory requirements.
Uses available information technology and other resources to support day-to-day work operations.
