DESCRIPTION: Under the general supervision of the Director of Pharmacy, the pharmacist is responsible for ensuring optimal medication treatment outcomes while providing cost-effective and safe medication therapy. The Pharmacist will review and interpret clinical appropriateness of medication orders and regimens and communicate any and all concerns with prescribers, and advise prescribers, nurses and other health care professionals on medication therapy decisions. They will be responsible for product verification prior to dispensing, therapeutic drug level monitoring, aseptic compounding of sterile products including antineoplastic and other hazardous medications, and verification of total parenteral nutrition orders, and extemporaneous medication compounding. The Pharmacist will dispense and maintain controlled dangerous substance inventory, prevent adverse drug events and near misses through sound application of knowledge and skills paying particular attention to duplication of therapy, appropriate dosing, drug interactions, and side effects. They will maintain accurate and complete documentation of interventions, adverse drug events and near misses, and will oversee the activities of pharmacy technicians. Operate all pharmacy automation.
ADDITIONAL REQUIREMENTS: BS or Pharm D Degree-New Jersey Pharmacy License. Prior hospital pharmacy experience preferred. Strong communication skills a must.
