DESCRIPTION: Clinical Generalist Evening Shift--Reviews patient profiles to assure appropriate therapy based on patient specific information that includes at least the following: age, gender, diagnosis, co-morbidities, relevant laboratory values, allergies, height, weight and BSA, pregnancy and lactation status. Reviews and transcribes all medication orders including specialized orders based on approved protocols accurately and timely verifying therapy, correct medication, route, dose and frequency. Makes recommendations for alternate treatments when necessary. --Participates in clinical initiatives. Initiates appropriate clinical interventions and clarifications with corresponding documentation. Demonstrates compliance with Pharmacy and Therapeutics Committee initiatives. Reviews pertinent information to monitor patients therapies and makes appropriate recommendations eliminating duplicate, unwarranted, or inappropriate therapy. Converts non-formulary medication requests to appropriate formulary alternatives whenever possible.--Provides drug information to physicians, nurses, other health care professionals, and patients/family members. Demonstrates and shares knowledge of various drug information resources and knows how to research and interpret data and information.--Associated responsibilities include but are not limited to:-Therapeutic drug level monitoring-Aseptic compounding of sterile products including antineoplastic and other hazardous medications-Total Parenteral Nutrition-Extemporaneous medication compounding-Medication usage area inspections-Unit dose repackaging-CDS dispensing and inventory maintenance-Oversight of pharmacy technicians-Mentor pharmacy students and residents-Operate all pharmacy automation--The above statement reflects the general duties considered necessary to describe the principal functions of the job as identified and shall not be considered a detailed listing of all work requirements.
ADDITIONAL REQUIREMENTS: Pharmacy Degree from an ACPE accredited university. PGY-1 Residency Preferred. New Jersey Pharmacy License. Prior hospital experience preferred. Familiarity with pharmacy automation preferred. Sterile and hazardous compounding preparation preferred. Strong communication skills a must.
