The Staff Pharmacist shall provide the necessary pharmaceutical services needed to facilitate the physician in the treatment of the patient. Such services shall include medication and IV dispensing, monitoring drug regimens including food/drug and adverse interactions, consulting with physicians and nursing staff concerning drug therapy, preparation of sterile admixtures, preparation of cytotoxic agents, and providing assistance during "Code Blue" codes. In addition, the pharmacist shall be responsible for managing technicians, for assuming management responsibilities of the department in the absence of Pharmacy Manager or Director, for managing inventory, and for supporting cost containment programs. Providing services to patients of all ages at a high competency level.
Handles physician orders appropriately.
Interprets and fills physician orders accurately and timely.
Checks for allergies, interactions, duplications, and appropriate dosing.
Confers with nursing and/or the physician when questions about medication orders arise.
Makes appropriate recommendations and documents as a pharmacist intervention.
Ensures quality pharmaceutical care.
Performs pharmacokinetics and protocol responsibilities.
Participates in codes.
Participates in performance improvement and drug utilization evaluations.
Participates in all clinical programs.
Makes appropriate recommendations concerning drug levels and dosing based on creatinine clearance.
Manages patient parenteral nutrition therapy through approved protocols.
Manages Hospital Resources.
Assists with appropriate maintenance of drug and supply inventory.
Assists with appropriate management of human resources by insuring productivity and flexing according to guidelines.
Assists in training of new technicians.
Checks unit dose prepackaging, stock fill, cart fill, and crash cart fill of technicians.
Checks utilization of Pyxis system in filling, adding to inventory, and using the reports.
Directs the work of the technicians during slow times to appropriate tasks or flexes the associate off.
Obtains knowledge of, and demonstrates compliance with infection control and safety policies and procedures.
Practices standard precautions in patient care activities.
Uses proper procedure in handwashing.
Wears appropriate personal protective equipment when preparing cytotoxic drugs.
Checks technique and preparation of intravenous admixtures of technicians.
Appropriately handles and disposes of sharps.
Use proper aseptic technique when preparing intravenous admixtures.
Complies with 797 regulations.
Complies with legal and departmental requirements.
Maintains a current Louisiana pharmacist license.
Completes pharmacy annual competency checklist.
Maintains security for control drugs.
Maintains documents required by Louisiana Board of Pharmacy, DEA, FDA, JCAHO, OSHA, and other regulating agencies.
Training in clinical pharmacy, IV compounding & hospital pharmacy perferred.
2 years pharmacy practice preferred. At least one year hospital pharmacy experience desired.
